Be completely honest and thorough in identifying your areas of need, as this process is all about you and your needs. Please fill out both forms if you are both a new client and a medical massage patient.

For all medical massage patients, download, print, and fill out the medical massage patient form before your appointment. If you are both a new and medical massage patient, complete both forms.
